After the talk, you will be able to fill out a medical history form and individual consent forms for whatever vaccines you choose to receive. In addition anti-malarial and antibiotic prescriptions will be available upon completion of a medical form (to be reviewed by a nurse.)

To fill out a medical form for review by a nurse - $25.00 (this is required for all vaccines to be administered and to write prescriptions).
