 Letter - January 13 Dear Student (and parents),
Thank you for including the Dominican Republic Mission Trip in your Spring Break plans. We have accepted your application (and your non-refundable deposit) but this is only the first stop in your journey. The next steps are listed below.
1. If you do not have a passport, apply now. We will not be responsible if yours does not arrive in time. Go to http://travel.state.gov/passport 2. January 25, 2009 @ NOON – Our first meeting, bring the following: 1. Parental permission, and consent for treatment form – MUST be notarized. 2. A copy of the picture page of your passport unless noted below. If you cannot be there – let Pastor Shawn or me know AND try to send a parent in your place. Parents are welcome at this meeting and we will do our best to answer any questions. 3. February 15, 2009 – Fifty (50) percent of cost is due or no ticket will be bought. 4. February 22, 2009 – Meeting (Ministry planning and training – Optional immunizations) 5. March 10, 2009 – Ninety (90) percent of trip cost is due. Note if you back out at any time funds are non-refundable due to their charitable donation status. 6. March 22, 2009 – Meeting and Packing of Supplies 7. March 25 – Be at BWI airport Delta Airlines by 5:00 AM
All meetings will take place in the Spencerville Church Youth Room. If you can’t make a meeting please contact Pastor Shawn or me so you will have all the necessary information. Forms, facts and information from every meeting are all at the website www.spencervilleyouth.com.
The enclosed Permission and Medical Release Form is one of the absolute requirements of every trip participant. It must be completed and returned by January 25, 2009.
A sample fundraising letter is on the website. We highly recommend you include a tear-off for your donors to include with their check. The tear-off should include your name, the amount of their donation and that the donation if for the Youth Mission Trip.
